About Us The Elbowoods Memorial Health Center (EMHC) is an American Indian Health Facility primarily serving the Fort Berthold Indian Reservation.
The Fort Berthold Indian Reservation is located in central North Dakota; home of the federally recognized Mandan, Hidatsa, and Arikara Nation otherwise known as the Three Affiliated Tribes.
The reservation sits along the banks of the Missouri River and consists of 988,000 acres which is the home base for approximately 16,700 enrolled members.

EMHC is owned and operated by the MHA Nation.
Historically, the healthcare was provided to the people of the MHA Nation by Indian Health Services.
In 2008, the MHA Nation's health care system became a PL 93-638 Ambulatory Health Care Center; therefore this facility is operated under the rules and regulation of the MHA Nation along with its accrediting organization Accreditation Association for Ambulatory Health Care, Inc.
EMHC has four additional field clinics each with a provider, nurse, pharmacy technician, and support staff.
Other associated clinics include Dental and the Awatii Wellness Center.
We also provide direct health services to native people from other federally recognized tribes; as well as inmates occupying the Justice Center.
Our general clinic services initiate referrals to consultants, prescribe medications, and formulates a medical plan for each patient.

Summary:
(position includes but is not limited to the following):
1)Position serves primarily as Licensed Clinical Social Worker for the Elbowoods Memorial Health Center within the Health Care System.
a)Job Summaryi) Is responsible for planning and implementing a community oriented health program for the clients of EMHC.
This includes integration of health concepts into the total medical and health program of the Health programs through the identification of psycho-social health needs of patients who are newly admitted to healthcare facilities,ii)Knowledge of socio-cultural factors that adversely affect the health of the population served and ability to design corrective actions for the improvement of these problems (i.
e.
, primary, secondary, prevention/community education) across the age span from childhood through adulthood.
iii)Conducts Depression screening for the patient diagnosed with, and the population at risk for developing healthcare complications.
Provides appropriate follow-up for the clients who are found to have Depression.
iv)Establishes support groups for patients on the Diabetes disease continuum.
v)Active participation with the Diabetes team in planning and implementing the psycho-social aspect of diabetes care.
vi)Provides training, information, and consultation for staff and community organizations regarding behavioral health as it relates to the patient with diabetes, families of diabetics, and people at risk for diabetes.
vii)Provides behavioral change counseling on an individual and/or group basis across the age span as appropriate.
viii)Develops behavioral health care plans and provides appropriate follow-up for clients.
ix)Other duties as assigned by Diabetes Program Director.
2)Represents TAT Health Care Centers in a highly professional manner.
3)Establishes positive communications with all departments of EMHC to assure stable operations.
4)Demonstrates respect and understanding of confidentiality for patients, staff and others according to policy and HIPAA regulation.
5)Maintain required reporting as assigned.
6)Participates in EMHC staff meetings, assigned committees, community events, and other meetings as instructed or deemed necessary.
7)Actively strives to educate the community on the mission, vision and values of EMHC in a positive approach to assist in the growth of the organization and assist our community in its healthcare needs.
8)Adheres to the Mission, Vision, and Values of the Elbowoods Memorial Health Centers.
Qualifications:
o Must hold a Masters Degree in Social Work (MSW), REQUIRED.
Must submit copy of degree or transcripts with application.
o Hold a current license from the North Dakota Social Workers Association Board, REQUIRED.
o LCSW - Licensed Clinical Social Worker, PREFERREDo Computer knowledge and proficiency, REQUIRED.
o Possession of a current, valid North Dakota State Driver's License, reliable transportation, and telephone, REQUIRED.
o Excellent verbal and written communication skills, REQUIRED.
o Ability to work independently in the absence of supervision.
o Must be responsible, dependable, and able to maintain confidentiality of information.
o Must submit to clear Criminal Records Background Check.
o Must submit to an Alcohol/Drug Screen and random testing as per policy.
o Indian Preference will apply.
